The following forms can be opened and printed on your computer system. You can fill out the forms needed and bring them into the office on your first visit. This will save you the time and trouble of filling them out in the office. You will always need the Client Bill of Rights and Intake Appraisal form on the first visit. The others are self explanatory. If you are unsure what forms you need, just call 508-478-8500 and we will help you.

It is the policy in this office that a physician’s approval is required before proceeding with some cases. This form is required if client is seeking help for an issue that is a medical condition and/or client is under a Doctor's care for this issue.
