Nutrition Education
Needs Assessment Survey

All elementary teachers received a survey from the University of Connecticut to determine what nutrition education lessons are being used in the Hartford Public Schools. This information will help us design a better nutrition education program for students. Please complete and return to Dr. Rafael Perez-Escamilla at the University of Connecticut and win the chance to have a nutrition puppet show at your school.
