Required Assessment
All educators requesting PDP's from NE/AOSA workshops are required to complete
the following assessment no later than June 1, 2013.
The completed assessment should include the following:
1. Educator's name, address, phone, email, and teaching position
2. PDP forms for all NE/AOSA workshops attended during the 2012-2013 series
A minimum of 10 hours is required
3. A self addressed stamped envelope
4. One of the following:
A lesson plan based on the materials from the NE/AOSA workshops you have attended. Grade level, learning objectives, and process should be included.
- or -
A printed program of actual performance containing materials from the NE/AOSA workshops you have attended.
Please return the completed assessment no later June 1, 2013 to:

NE/AOSA Professional Development
Educators will receive documentation of their total PDP's from NE/AOSA.
All educators should retain a copy of their assessment for their personal files.
In the event of an audit, NE/AOSA will keep all assessments on file for the duration of the recertification cycle.
